to request a repeat prescription
You may request repeat prescriptions until your doctor requests a
review.
All requests must be in writing and can be brought or posted to the
surgery accompanied by a stamped and self-addressed envelope or by
faxing the surgery on 01446 733555.
Please tick your repeat prescription list; this is printed every time
you are issued with a new prescription. It is the half not retained by
the pharmacist.
Sometimes your doctor will not authorise the repeat prescription and ask
for an appointment review.
Requests for repeat prescriptions will not be taken over the telephone
as errors may occur.
Please allow 48 hours and any postage time to avoid running out of
medication.
