Are you in need of healthcare advice?

If you have toothache, sore or bleeding gums, troublesome wisdom tooth, tooth sensitivity or other types of facial pain your Primary Choice is a Dentist

If you have problems with your eye health like injuries, pain, swelling, visual loss or disturbance your Primary Choice is an Optometrist

If you have a minor illness like coughs, colds, diarrhoea or headaches or need advice on medications your Primary Choice is a Community Pharmacist

If you call your GP Practice you will speak to the GP Receptionist. They are trained to ask questions about your healthcare needs to help signpost you to the most appropriate person to provide advice or treatment

Your General Practice Nurse is your Primary Choice for wound and minor injury treatment, contraception advice and reviews, immunisations, blood monitoring and chronic illness reviews.

By making the right Primary Choice means your GP can spend time with the most vulnerable, complex and very ill patients.

Patient Registration

If you live in the Practice area and wish to register for NHS general medical, maternity or contraception services, please telephone us or call in. 

Please check the Practice Boundary page to see whether you reside within our catchment area for both West Quay and Rhoose. 

You will need to complete a registration form (GMS1W) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be found below.  We require two forms of identification, one with photo ID and one with proof of address.  Your NHS number is also required to register, this can be obtained from your previous surgery.

Patients are no longer allocated to a specific doctor; whilst you may be registered in the name of a particular Partner GP, you will be seen by any Practice GP.

When you register with us we think this is a good opportunity to discuss your health and get to know your medical problems and requirements if there are any. We want to not only treat any known conditions, but also provide advice on how to stay fit and healthy wherever possible.
